Introducing Strive Health

We understand that managing care for conditions like end-stage renal disease (ESRD) and chronic kidney disease (CKD) can be stressful and complicated. That’s why Capital Blue Cross is working with Strive Health to offer you a program that helps coordinate your kidney care.

When you sign up for Strive Health, you'll be supported by your own team of Kidney Heroes™ — a compassionate group of nurse practitioners, care managers, and other professionals who specialize in kidney care. Your Kidney Heroes team will also work closely with your primary care provider (PCP) and nephrologist (kidney doctor) to keep them informed of your condition and the services given to you.

How Strive Health can help

Your Kidney Heroes team will provide support so you can make the most informed decisions for your health. As a participant in the Strive Health program, you will:

  • Work with your Kidney Heroes team to understand kidney disease, what you can expect from this program, and how you can best manage your overall health.
  • Get education from dieticians, clinicians, and social workers to support your personal needs face to face1 and online.
  • Benefit from more quality time with your care team and get your questions answered.
  • Be able to connect with the right specialists to address any healthcare needs.

How to enroll
