Capital Blue Cross works with HarmonyCares to provide in-home medical care for eligible Medicare Advantage members with complex healthcare needs. These patients may have difficulty leaving home or accessing care in a traditional medical setting. Our aim is to help them manage their health, receive needed care and diagnostic services in their homes, and reduce their risk for poor health outcomes.
HarmonyCares is designed to support and strengthen the relationship between you and your patients.
HarmonyCares can collaborate with you to:
- Identify and address care gaps.
- Support chronic disease management.
- Complete medication reconciliation.
- Provide post-discharge follow-up.
- Coordinate care and connect your patients with social work services.
- Deliver mobile lab and palliative care services.
- Improve access to care for patients who have difficulty leaving home.
- Give your patients 24/7 access to a clinician through an urgent care line, reducing emergency room visits and hospitalizations.
HarmonyCares keeps you at the center of your patient’s care
Shares visit summaries, communicates significant clinical changes, and coordinates with provider offices following hospitalizations.
Communicates using each provider's preferred method, including fax, phone, or secure messaging.
Provides additional support for high-risk patients between office visits.
Offers convenient access for patients with mobility or transportation challenges.
Supports care coordination and continuity.
Does not redirect patients from their primary care provider; its goal is to support and reinforce the existing patient-provider relationship.
